ABSENCES

Absences are strongly discouraged during term time, however if an absence is unavoidable please complete and return the appropriate request for leave form.

Request for Leave forms

We ask that Parents (where ever possible) arrange for medical appointments and holidays to be taken outside of normal school hours.  However if you are unable to avoid taking leave during school time please complete and return a request for leave.  Please make sure the request for leave is submitted at least two weeks prior to the planned absence.  Your request will be reviewed using our absence policy and you will be contacted in writing to confirm if the leave would fall into the authorised or unauthorised absence reporting category.

Medical Forms

If your child has or develops a medical need we will support their attendance at Nursery in the best way possible for your child.  For a long term medical need it is important that staff are aware of the diagnosis, signs and symptoms to look out for and if any medication or emergency care is needed.  Similarly if your child develops a short term illness that requires medication to be administered by the Nursery setting, we will require you to complete a short term medical form.

ALL medication must be prescribed by an authorised pharmacy with the prescription label showing the childs name, dosage and frequency. 

Short Term - provides us with authorisation to administer prescribed medication for a short period e.g. antibiotics for an infection.

Long Term - provides us with authorisation to administer medication required for a long term condition or in a potential emergency.  e.g, asthma pump, epi-pen, insulin etc.

Food Allergy/Intolerance

FOOD ALLERGY

We work with our caterers to ensure that any confirmed food allergies are accounted for in our lunch and tea menu's in order to do amend our menu's and to make sure that we are fully prepared for any potential reactions we need to have all of the information relating to your childs allergy.  This includes a copy of your Allergy Information sheet supplied by your GP or health care professional along with the two forms on the right. 

If your child requires an epipen or antihistamine to be kept on site you will also need to complete and return the Long Term Health Care Plan.
